Contractor prequalification form

The pre-qualification application for contractors must be completed in full in order to be considered an Approved Contractor for Astra Group. This document is intended to provide information on the capacity, skill and expertise of the contractor. Applicants may supplement information requested with additional sheets, if required.

Astra Group reserves the right to add, delete and change Approved Contractors at any time in their sole discretion without notice. Completion of this form does not render the contractor an Approved Contractor nor entitle the contractor to perform work without prior approval from Astra Group.

Before filling out the application you will need to following information:

1. Copy of liability insurance meeting the following requirements:

   a) Certificate Holder & Additional insured:
   Astra Energy Services Corp c/o Astra Group & Astra Construction Management
   200, 638 11 Avenue SW
   Calgary, Alberta T2R 0E2

   b) Commercial general liability insurance for not less than $2,000,000.00 for death or injury to one or more persons and for property damage for each occurrence.
   The certificate is to contain a waiver of any rights of subrogation against Astra Energy Services Corp, Astra Group, Astra Construction Management & their clients.

   c) Certificate is to contain confirmation that the insurer will provide the Certificate Holder with 30 days’ notice of cancellation of the policy or of any material change in the policy;

2. WCB Clearance letter
3. Banking information (account, transit, institution, address, ) for EFT transfers
4. Bonding info if applicable
